According to troopers investigating the crash, Timothy O'Brien, 43, of Wakeman, was westbound on Ohio 113 in a 1994 F350 dump truck when he stopped to turn left into a private drive at 6:18 p.m.

Michael Durkalec, 41, of Wakeman, was also traveling west on Ohio 113 in a 2005 Subaru Forester. Durkalec failed to slow down behind the dump truck and attempted to pass it. He side-swiped the truck and drove left of center, striking an eastbound 2006 Ford F150 head-on.

The Subaru came to a rest in the middle of the roadway. The Ford F150, driven by Brent Leimbach, 33, of Amherst, went off the south side of the road and came to a rest in a yard.

Leimbach was trapped in the truck and had to be extricated by medical personnel. He was taken to Cleveland Metro hospital with non-life threatening injuries.

Durkalec was flown by medical helicopter to Cleveland Metro hospital where he is in "extremely critical condition," according to troopers.

O'Brien was not injured.

The crash remains under investigation. Seatbelt use and alcohol or drug use are unknown at this time.
